乙肝患者肝移植前后血清病毒指标的变化及意义分析

目的观察乙肝患者肝移植术后血清5项指标及HBV DNA含量的变化并分析其临床意义。方法选择2006年10月-2007年3月在解放军第302医院施行肝移植术且术前采用拉米夫定治疗的35例乙肝患者为研究对象,采用ELISA法测定手术前、后血清5项指标,采用荧光PCR法测定血清HBV DNA含量变化。选择拉米夫定治疗前HBV DNA高于10000U/ml的45例乙肝患者作为对照,测定用药前和用药2周后血清乙肝5项指标和HBV DNA。结果35例肝移植患者中移植后第2天,除1例术前、术后乙肝5项和HBV DNA无明显变化外,其余34例肝移植患者的血清HBsAg和HBV DNA全部转阴,HBsAb全部转阳。其中11例大三阳患者的HBeAg全部转阴,6例发生从HBeAg到HBeAb的血清学转换,但移植前后血清HBcAb均无变化。45例拉米夫定对照患者均未发生HBsAg阴转和HBsAb阳转的现象,11例患者血清HBV DNA含量下降到500U/ml以下,29例血清HBV DNA含量下降,但持续高于1000U/ml,5例患者的HBV DNA无明显变化。在34例大三阳患者中,有29例患者HBeAg转阴但未发生HBeAb阳转现象。结论肝移植术外加抗病毒和免疫学的综合治疗方案,可使97%的乙肝患者彻底清除血清HBV DNA,并发生HBsAg、HbeAg的血清转换。

Kinetic characteristics of continuous flow polymerase chain reaction chip: A numerical investigation

Continuous flow PCR (polymerase chain reaction) chip holds impressive advantages compared to micro chamber PCR chip. In order to have better understanding of kinetic characteristics of continuous flow PCR chip, a comprehensive mathematical model is presented in this paper, including melting, annealing and extension phases of a typical PCR process which has the essence of a convection-diffusion-reaction system. Using this model, we can simulate the PCR process in series of reaction cycles. Numerical results show that the average sample velocity plays a significant role in affecting the amplification efficiency. Also, appropriate combination of the PCR mixture is important for high-quality DNA amplification. Giving a large initial DNA concentration range, the continuous flow PCR scheme holds excellent real-time detection ability theoretically. The present numerical model bridges the temperature distribution to the real DNA amplification, and thereby is able to successfully predict continuous flow PCR properties which are important for the chip design.

慢性乙肝患者口腔隐血与唾液HBV DNA水平的关系被引量:3

目的:探讨不同口腔隐血状况下慢性乙肝患者唾液HBV DNA的阳性率和病毒浓度。方法:荧光定量聚合酶反应PCR法和隐血检测研究60名慢性乙肝患者唾液中HBV DNA的检出率和病毒含量以及唾液隐血状况。结果:唾液DNA阳性率在唾液隐血阳性和弱阳性者间为62.5%,唾液隐血阴性者为63.6%。结论:慢性乙肝患者唾液中有无血液污染对唾液HBV DNA的水平无明显影响。

Hybrid microsystem with functionalized silicon substrate and PDMS sample operating microchannel: A reconfigurable microfluidics scheme

A hybrid microsystem with separately functioned temperature controlling substrate and sample operating fluidic microchannel was developed to demonstrate a reconfigurable microfluidics scheme.The temperature controlling substrate integrated a micro heater and a temperature sensor by using traditional silicon-based micromechanical system(MEMS)technique,which guaranteed high performance and robust reliability for repeatable usage.The sample operating fluidic microchannel was prepared by poly-(dimethylsiloxane) (PDMS)based soft lithography technique,which made it cheap enough for disposable applications.The PDMS microchannel chip was attached to the temperature controlling substrate for reconfigurable thermal applications.A thin PDMS film was used to seal the microchannel and bridge the functionalized substrate and the sample inside the channel,which facilitated heat transferring and prevented sample contaminating the temperature controlling substrate.Demonstrated by a one dimensional thermal resistance model,the thin PDMS film was important for the present reconfiguration applications.Thermal performance of this hybrid microsystem was examined,and the experimental results demonstrated that the chip system could work stably over hours with temperature variation less than 0.1oC.Multiple PDMS microchannel chips were tested on one heating substrate sequentially with a maximum intra-chip temperature difference of 1.0oC.DNA extracted from serum of a chronic hepatitis B virus(HBV)patient was amplified by this hybrid microsystem and the gel electrophoresis result indicated that the present reconfigurable microfluidic scheme worked successfully.

SIMPLIFIED SCALING TRANSFORMATION FOR THE NUMERICAL SIMULATION OF MEMS DEVICES WITH THIN FILM STRUCTURES被引量:1

Thin film is a widely used structure in the present microelectromechanical systems (MEMS) and plays a vital role in many functional devices. However, the great size difference between the film's thickness and its planar dimensions makes it difficult to study the thin film performance numerically. In this work, a scaling transformation was presented to make the different dimensional sizes equivalent, and thereby, to improve the grid quality considerably. Two numerical experiments were studied to validate the present scaling transformation method. The numerical results indicated that the largest grid size difference can be decreased to one to two orders of magnitude by using the present scaling transformation, and the memory required by the numerical simulation, i.e., the total grid number, could be reduced by about two to three orders of magnitude, while the numerical accuracies with and without this scaling transformation were nearly the same.

硅对聚合酶链式反应抑制作用的实时定量实验研究

采用实时定量PCR的方法研究了硅表面对乙肝病毒(HBV)DNA扩增过程的抑制作用。实验中采用HF处理的H终止硅和与自然氧化的硅片作为样品,按照1.0mm2/μL和5.2mm2/μL的面体比(硅片总表面积与PCR反应液的体积之比)与PCR反应液预混合(未加DNA样品,同时,TaqDNA聚合酶的浓度分别为0.2U/35μL和1.0U/35μL),充分混合预设的时间后,取出硅片,汲取上清液与DNA样品混合后采用实时定量PCR仪SlanTM进行扩增。扩增结果的荧光曲线表明:自然氧化的硅样品对PCR抑制作用更强,其对Taq聚合酶的抑制效果大于4.4mU/mm2;高面体比条件下,即使在初始扩增阶段,也达不到理想的指数形式;缩短芯片反应时间有利于降低材料对扩增的抑制效应。
